The parents or teacher may not identify a depressive problem for what it is, and young children should not be expected to diagnose themselves. While we may not often recognize depression in young children either because it occurs infrequently, as suggested by the Oregon study, or because we are not sophisticated enough to identify it when it does occur — the situation is quite different for adolescents. Depression in adolescence is a widespread problem. Suicide among teenagers, for example, has received a great deal of publicity and for good reason.

The problem with this approach is that a very large number of people who are depressed do not seek professional treatment. Moreover, the people who do seek treatment are not likely to be representative of the entire population of depressed people; they are likely to be more affluent and better educated. Minority groups will be underrepresented. Probably the only way to estimate how many people are depressed in the United States is to go knocking on doors in our cities, towns and farms, to survey representative samples of our population and to ask questions that will accurately tell whether a person is depressed.

Reports of current major depressive episodes were three times as frequent in the urban county. When the researchers controlled statistically for a number of important social and demographic factors, such as sex, education, race and selective migration, they continued to find a substantial difference in the rates of depression between the urban and rural areas. 19 The tendency observed in the North Carolina study for residents of urban areas to more frequently report being depressed than residents of rural areas has been observed in other countries.
